In this section we have provided you with access to forms used in our office and that may be needed by students and parents throughout the school year.

Student Brag Sheet

Students who need a letter of recommendation, be sure to fill out a brag sheet with as many details and specifics you can. Remember this letter is to highlight all of your accomplishments so details matter.
Teacher Evaluation

FOR TEACHERS – Along with a Brag Sheet, for a letter of recommendation students should have two teachers complete evaluations for them. You can tell them to access that here, or get a sheet of paper from your counselor to give the teachers.
Hospital Homebound (HHB)

If you have had a discussion with a school counselor or an administrator about HHB services, please use this form to it’s entirety and sent/faxed to Dr. Christina Lowe at our county’s board of education office. If you're student has an IEP and is served in Special Education, the form must be sent to Dr. Brad Bowling.
Parent/Guardian Rights

Have you recently had a 504 Plan meeting with teachers and counselors? Here is the electronic copy of what was given at the meeting in case you have lost your copy.
Former Graduate Records
